Home health can include care given in the home by skilled medical professionals (clinicians) including, skilled nursing, physical therapy, occupational therapy, and speech therapy.

Home health can also include non-medical care, such as social services or assistance with daily living from a home health aide. The goal of home health is to help patients get better, reach a level of independence, become as self-sufficient as possible, and reduce the risk for unnecessary hospitalizations.

Hospice is not a place, rather, a type of care focused on the patient and their needs. Our Hospice service is designed to come to you; whether it is a single-family residence, assisted living, nursing home, or apartment.

The goal of hospice care is to provide physical, mental, emotional, and spiritual comfort for those who are terminally ill.

Home Care and Hospice Services

Medicare provides home health and hospice services to patients who are homebound and need assistance caring for themselves at home. These services if aligned correctly to the patient's current condition increase quality of life and promote comfort. Home Health is a program that is 100% covered by Medicare and focuses on an individual's ability to rehabilitate. If a patient does not have rehabilitative potential and experiences uncontrolled symptoms and frequent rehospitalization and doctors visits they may be more appropriate for Hospice.

Hospice is generally misunderstood by the public but if understood correctly provides great benefits to patients and their families lives. When patients have attempted Home Health and have failed to reach goals or continue to decline physically they may be appropriate for Hospice. Other scenarios when patients may be appropriate for Hospice Care are when patients have decided to stop seeking curative treatments or have a terminal diagnosis that is significantly limiting their life and quality of life.

Home Health and Hospice are highly complimentary service lines. If a patient does not qualify for Home Health Care they may be appropriate for hospice as explained above. If patients that are currently receiving Hospice Care begin to recover they could once again qualify for Home Health Care.

Our Home Health Process

1. Whether in a hospital or at home, a patient needs a physician order to be written to receive home health services.

2. The patient’s physician will document diagnoses and patient history that will show the patient meets medical requirements to be admitted for services. Lighten will work directly with the Doctor to ensure the proper documentation is provided.

3. Our intake coordinator will verify insurance benefits to ensure the patient is covered to receive home health services.

4. Our scheduler from Lighten will call the patient and/or family to set up the initial evaluation in the patient’s place of residence with a Lighten nurse case manager and/or physical therapist.

5. During the initial evaluation, the nurse case manager/physical therapist will determine the frequency of visits and, with the patient, create a personalized plan of care.

Our Hospice Process

1. In order to be admitted for hospice services, a patient must first meet hospice eligibility requirements determined by the patient’s primary care physician.

2. The patient will receive an order to be admitted to hospice by their primary care physician. Lighten representatives will work with the Doctor to ensure proper documentation is provided.

3. Once the order is received, a representative from Lighten will call the patient and/or family to set up the initial evaluation in the patient’s place of residence with a Lighten nursing case manager.

4. A Lighten nurse case manager will come to the patient’s place of residence to perform the initial assessment who will help create a personalized plan of care focused on the needs of the patient.
