Meet Anita Lane, a lifelong Jazz fan who has an incredible knack for knowing the outcome of every Jazz game before the opening tip! For years, she has kept a notebook filled with the score of every Jazz game. Believe it or not, Anita has NEVER been to a game! It was a dream of hers to attend… so we got to work.
Our team reached out to the Jazz and told Anita’s story. The Jazz graciously offered her tickets to a playoff matchup against the Rockets. Although she feared her physical limitations would keep her from attending, she was determined.
Our team coordinated the availability of a wheelchair for her use. Transportation was arranged to take Anita and her nurse, Miwa Skowronek, to and from the arena. “She was so excited to attend the game. She was so happy,” recalls Ms. Skowronek. We believe in helping our patients maintain their maximum level of mobility and independence. What’s important for patients to understand is the degree of assistance they may be eligible for in the comfort of their own home.
Our clinical staff is trained to assist with a wide range of illnesses and conditions. Whether you had recent surgery, need help managing your pain, or struggling with a chronic illness, our team of clinicians is here to help. Our clinical staff consists of Registered Nurses, Physical Therapists, Occupational Therapists, and Speech Therapists who will assist you in creating a personal care plan centered around your improvement.
We coordinate every step of care with your Primary Care Physician to ensure every need is met. You will have a personal Case Manager who will make frequent in-house visits and will always be a phone call away to answer any questions you may have.
Hospice: Helping You Get the Most Out of Life’s Journey
We know how hard the end of life’s journey can be. At Lighten, we serve to make your burden lighter. It is why our goal is to ensure our patients and their families feel that every measure is taken to promote comfort and well-being.
Hospice is not a place, rather, a type of care focused on the patient and their needs. Our Hospice service is designed to come to you; whether it is a single-family residence, assisted living, nursing home, or apartment. The goal of hospice care is to provide physical, mental, emotional, and spiritual comfort for those who are terminally ill.
Each patient will have a Registered Nurse as their personal Case Manager in charge of overseeing their care. Many ancillary services are provided with no extra cost to the patient or their families including medical equipment, medication, spiritual care, Dietitian Consultant, Social Worker support, and much more.

Lighten Home Health and Hospice serves patients up and down the Wasatch Front including Utah, Salt Lake, Tooele, Davis, and Weber Counties. We have nursing staff on-call 24/7 who are able to help and assist you with anything you may need.
